Heartland Hospice

The Mission of Heartland Hospice is to provide quality Hospice Care to patients with a limited life expectancy, as well as, provide support to their caregivers. The quality of life of those we serve is enhanced through palliative and specialized supportive care designed to meet the physical, psychosocial, social, and spiritual needs of the patients and caregivers. Hospice care is provided by using an interdisciplinary team utilizing an approach to the care delivered to patients either in their own home or extended care facility; working cooperatively with area health care providers and other community resources. Heartland Hospice strives to provide care to all who are eligible and need Hospice care irrespective of ability to pay.

Special Projects Volunteer/Arts and Crafts

Starts: March 1st, 2009

Groups can volunteer together at this opportunity.

Volunteers must be at least 16 years old.

Description
You will work with facility Activities Director to facilitate arts and crafts projects. You may do this individually or as a team of 2-3. You may also design and create your own activities, (per approved by Activities Director).

Volunteers must be at least 16 years old. Complete an application and two references. Interview prior to any training or orientation. Complete a mandatory 4 yours or Orientation and all required pre volunteer documentation. Sign confidentiality agreement. Complete a criminal history and background check.

Office Volunteer/Preparing Admission Packets

Starts: March 1st, 2009

Volunteers must be at least 16 years old.

Description
Under direction of Volunteer Coordinator you will: Gather, collate, prepare, and copy paperwork used in Admission Packets for Hospice Team Training and familiarizing you with forms an order of packet will be provided. Must do most of this within certain time constraints. Done in main office.

Volunteers must be at least 16 years old. Complete an application and two references. Interview prior to any training or orientation. Complete a mandatory 4 yours or Orientation and all required pre volunteer documentation. Complete mandatory 2 Step PPB (TB test). Sign confidentiality agreement. Office Volunteer/Special Projects

Starts: March 1st, 2009

Volunteers must be at least 16 years old.

Description
Assisting with Volunteer Booth and Health and Opportunity Fairs. Under direction of Volunteer Coordinator you will: Staff Volunteer Department booth at outside functions. Answer questions regarding Volunteer opportunities and Volunteer Department. Training and support will be provided. This will be on-call but within known scheduled events.

Description
Create, design and make all occasion cards to mail to clients, caregivers, and volunteers. All Holiday, Birthday, Condolence, Get Well, all occasion even bland note cards for clients to personally send to friends and family. Volunteer provides all materials; no supplies are included or reimbursed. You may do this individually or as a team.

Volunteer/Running Errands/Making Deliveries

Starts: March 1st, 2009

Volunteers must be at least 16 years old.

Description
Under direction of Volunteer Coordinator or caregiver you will: Make a trip to the grocery, pharmacy, etc for caregiver when they cannot leave. Pick-up supplies and deliver to client's home. This project may be on-call or on a regular basis depending on need. Must do most of this within certain time constraints.

Friendly Visitor/Volunteer Visitor, Companion, Respite Provider

Starts: March 1st, 2009

Volunteers must be at least 18 years old.

Description
You will maintain a professionally compassionate relationship with client and family. Demonstrate interest and concern for the client and caregiver. Foster communication by involving hospice team. Be prepared to listen without judging or criticizing. Offer caregiver opportunities for exercise and social contacts. Report signs of economic hardship or need for professional counseling to volunteer coordinator or member of hospice team.

Special Needs Volunteer/Licensed Professional

Volunteers must be at least 18 years old.

Description
Provide specific licenses service/s. Beautician, Manicurist, Massage therapist, Counselor, Social Worker, Lawyer, Chaplin, CPA, Registered or licensed Nurse, and Home Health Aide. This will be scheduled at your availability.

Description
You will answer phone, transfer calls, and take messages for office staff. Training will be provided.

Description
Mailing of bereavement newsletters and follow-up letters. Under direction of Bereavement Coordinator you will: copy, stuff, label, and post Bereavement mailings. Envelopes, newsletters, mailing dates and intructions will be provided. Some follow-up calls maybe needed. Must do most of this within certain time constraints (may be done from home or office).
